and engaging opportunities for youth to gain spiritual growth, fellowship and fun at
UPC. If you are in Middle School or High School, please come join us!

Permission Form: There is a two-page permission and medical information form that parents must complete
and return to the church each year before their children can participate in church-sponsored events. Parents, please make sure
